Empowering individuals with physical disabilities to live independently
Our physical disability support service is designed to empower individuals with physical disabilities to live as independently as possible in their own homes. We provide practical, person-centred support that focuses on maximising ability and promoting quality of life.
Our caregivers are trained to work with individuals with a wide range of physical disabilities, including those resulting from neurological conditions, acquired injuries, and long-term health conditions. We take a strengths-based approach, supporting individuals to do as much as they can for themselves.
We work closely with occupational therapists, physiotherapists, and other healthcare professionals to ensure our care complements and supports the individual's overall rehabilitation and independence goals.
Physical disability support is for individuals of all ages living with physical disabilities, including those with neurological conditions such as MS or stroke, acquired injuries, cerebral palsy, and other long-term physical health conditions.
A detailed assessment is conducted to understand the individual's specific disability, functional abilities, and support needs.
We liaise with the multidisciplinary team, including OTs and physiotherapists, to ensure our care aligns with rehabilitation goals.
A detailed support plan is created that maximises independence and addresses all identified needs.
Our trained caregivers provide consistent, skilled support, with regular reviews to adapt to changing needs.
